Your Rights as a Patient
At Achievers ABA Therapy, every patient and family has the right to: be treated with respect and dignity at all times; receive services in a safe environment, free from abuse, neglect, or mistreatment; participate in decisions about your child’s care and treatment plan; be informed about the qualifications and credentials of your providers; provide informed consent before services begin and withdraw from services at any time without penalty; access your child’s medical records; receive clear information about fees and billing; maintain privacy and confidentiality of your health information; experience continuity of care and timely access to services; receive services using the least restrictive procedures appropriate for your child’s needs; file a complaint or grievance without fear of retaliation.
Your Responsibilities
We ask that patients and caregivers: provide accurate and complete information at intake and throughout services; attend scheduled sessions and communicate promptly about cancellations or concerns; respect staff, organizational policies, and clinic rules; adhere to the treatment plan as agreed upon with your provider; meet financial obligations for services rendered.
Complaints & Grievances
Submit complaints confidentially to feedback@achieversaba.com. All grievances are reviewed by our leadership team. You will not face retaliation for submitting a complaint. If you are not satisfied with our response, you may contact your health plan or the Indiana or Colorado state regulatory agencies overseeing ABA services.
