Your Privacy Rights
Achievers ABA Therapy is committed to protecting the privacy and security of your health information. This Notice describes how we may use and disclose your Protected Health Information (PHI) and your rights regarding that information, in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
How We Use Your Information
We may use and share your health information to provide and coordinate your care, obtain payment for services rendered, and carry out our health care operations. We will not use or share your information for any other purpose without your written permission, except as required or permitted by law.
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Your Rights
You have the right to: see and receive a copy of your health records; request corrections to your health information; receive a report of when and why your information was shared; request restrictions on how your information is used; file a complaint if you believe your privacy rights have been violated.
How to File a Privacy Complaint
You may file a complaint with us at feedback@achieversaba.com or with the U.S. Department of Health and Human Services Office for Civil Rights at www.hhs.gov/ocr. We will not retaliate against you for filing a complaint.
